If you have paperwork that needs to be completed by one of our doctors, your doctor may need to see you for an office visit to complete the necessary paperwork.
If the doctor is able to complete the paperwork without an office visit, we charge a fee of $75 to compensate the doctor for their time, and you will be billed directly for this fee. If the paperwork requires extensive time, we will inform you of any additional fees before the paperwork is completed.
Examples of paperwork requests include: life, disability, long-term care and other insurance paperwork; work excuse letters; handicap parking applications; prescriptions for, or other documentation of, over-the-counter medicines for Health Savings Reimbursement accounts.
